About Our Career Opportunity

Summit Strategic Solutions, a division of Summit Medical Group, has an immediate opportunity for an experienced Primary Care Behavioral Health Provider, Licensed Clinical Social Worker, LCSW or Psy.D./Ph.D. The Primary Care Behavioral Health (PCBH) Provider: (1) operates in a consultative role within primary care team, utilizing the integrated PCBH model; (2) provides recommendations regarding behavioral interventions to the referring PCP and; (3) conducts brief interventions with referred patients on behalf of the referring PCP. This is afull time opportunity, M-F, 8am-5pm.

Examples of Duties (List does not include all duties assigned)

1) Delivers brief, consultation services to patients and primary care providers using an integrated PCBH model.

2) Works closely with primary care providers and staff to develop and implement treatment plans.

3) Provides effective curbside consultations with primary care providers.

4) Provides screenings, assessments and interventions.

5) Works with patients to identify health strategies that are problem-focused and solution-oriented.

6) Encourages providers to refer patients via the "warm hand-off" approach to drive patient engagement and accountability.

7) Completes concise assessment, intervention and recommendation and management plan in EMR in a timely manner.

8) Triage / refer to specialty care providers as appropriate.

9) Completes risk assessments as needed.

10) Performs related work as assigned.


• Minimum of three years clinical experience as a behavioral health provider in a health care setting required.

• Previous experience in a primary care setting preferred.


• Licensed Clinical Social WorkerLCSW or Psy.D./Ph.D.

• Must possess all licenses, registrations, certifications, and accreditations required by law to render health care services in the state of Tennessee.
